Non-medical support for everyday life at home. Our caregivers assist with personal care, daily routines, meals, companionship, respite, and community activities while supporting each person’s independence, preferences, and goals.
Support beyond the front door.
COMMUNITY ACCESS & SUPPORT
We support eligible Community HealthChoices (CHC) participants with appointments, shopping, and community activities. Services may include caregiver accompaniment and coordination of authorized transportation. We do not provide transportation directly. Availability depends on authorization and staffing.
